Hospice Visit Number Affects Ability to Die at Home - Death & Dying

WEDNESDAY, June 27 (HealthDay News) -- Hospice patients with cancer are more likely to be able to die in the setting of their choice if they receive at least one hospice visit per day during the first four days of hospice care, according to research published online June 25 in the Journal of Clinical Oncology.

Neha Jeurkar, M.D., of the University of Pennsylvania Perelman School of Medicine in Lancaster, and colleagues conducted a retrospective, electronic health record-based cohort study involving 7,391 patients in three hospice programs in Pennsylvania, Florida, and Wisconsin. The authors sought to determine preferences and factors associated with an increased likelihood for cancer patients to die at home.

The researchers found that patients who preferred to die at home were more than twice as likely to die at home. Factors associated with an increased likelihood of patients who preferred to die at home actually dying at home included having at least one visit per day in the first four days of hospice care, being married, and having an advance directive. Cancer patients with a higher functional status or who were experiencing moderate or severe pain were significantly less likely to die at home.

"The results of this study add to our understanding of the factors that facilitate death at home and highlight the importance of early visits by hospice team members," the authors write.

Two study authors are employed by Suncoast Solutions, which developed the electronic health record used by the hospices in the study.

Scientists at the Mayo Clinic believe an enzyme known as Kallikrein 6, abundant in multiple sclerosis (MS) lesions, may be neutralized by an antibody which then could slow the demyelination process characteristic of the neurodegenerative disease.

Previously, researchers in the Department of Physical Medicine and Rehabilitation at the Mayo Clinic found an enzyme called Kallikrein 6, a secreted serine protease with diverse physiological functions. The same team, led by Isobel Scarisbrick PhD, have found an antibody that may neutralize Klk6, thus slowing MS progression. The scientists evaluated the process in mice infected with the Theiler's murine encephalomyelitis virus (TMEV). This virus induces encephalomyelitis comparable to multiple sclerosis. The model is based on the theory that viral infection early in life results in eventual abnormal immune response in the brain and spinal cord.

One week after being infected with the virus, the mice had elevated levels of Klk6. However, the neutralizing antibody blocked the enzyme, and the mice also had reduced inflammatory white blood cells and slowed depletion of myelin basic protein.

?These findings suggest Kallikrein 6 plays a role in the inflammatory and demyelinating processes that accompany many types of neurological conditions,? says Dr. Scarisbrick. ?In the early chronic stages of some neurological diseases, Kallikrein 6 may represent a good molecule to target with drugs capable of neutralizing its effects.?

Have you seen Maroon 5's newest video for "One More Night" yet? MTV News was lucky to have frontman Adam Levine and lead guitarist James Valentine stop by to premiere the clip on MTV during "MTV First: Maroon 5" on Monday, during which we learned that, from the band's point of view, love is battlefield. Or a boxing match, rather.

The concept for "One More Night" was dreamed up by Levine and directed by "Friday Night Lights" and "Battleship" director Peter Berg. It revolves around a boxer dealing with the pressures of providing for and being there for his young wife and baby. The young wife in this case is played by the lovely Minka Kelly, who worked with Berg on "Friday Night Lights." Although Levine and Kelly look every bit a happy couple at the beginning of the video, their romance is not of the fairy-tale variety.

"Minka was kind enough to make an appearance. That was pretty sweet," Levine said of the actress' involvement. "She plays my wife who leaves me." Spoiler alert, Adam! Luckily for everyone involved, the story behind getting Kelly to sign onto the project was a much happier tale than that of the two characters Levine and Kelly play in the video. It was as easy as picking up the phone and making a call.

"She's great," Levine said of Kelly. "The [story behind her involvement] was Pete reaching out to her and saying, 'Hey, do you want to do this?' and her saying 'Yes,' and us saying, 'Thank you!' " Levine explained, before giving her a full-on shout-out. "Thank you, Minka Kelly, for doing our video!"

In addition to her work in the cult hit "Friday Night Lights," Kelly has continued to appear on TV and in film with roles on "Parenthood" and the rebooted "Charlie's Angels," as well as on the big screen in "The Roommate" and a bit part in "(500) Days of Summer." Next up for Kelly is the rumored role of a young Jackie Kennedy in the upcoming ensemble "The Butler" alongside Forest Whitaker, Oprah Winfrey, Matthew McConaughey, John Cusack, Alan Rickman, Jane Fonda, Cuba Gooding Jr. and Lenny Kravitz.

Google simulates the human brain with 1000 machines, 16000 cores and a love of cats

Don't tell Google, but its latest X lab project is something performed by the great internet public every day. For free. Mountain View's secret lab stitched together 1,000 computers totaling 16,000 cores to form a neural network with over 1 billion connections, and sent it to YouTube looking for cats. Unlike the popular human time-sink, this was all in the name of science: specifically, simulating the human brain. The neural machine was presented with 10 million images taken from random videos, and went about teaching itself what our feline friends look like. Unlike similar experiments, where some manual guidance and supervision is involved, Google's pseudo-brain was given no such assistance.

It wasn't just about cats, of course -- the broader aim was to see whether computers can learn face detection without labeled images. After studying the large set of image-data, the cluster revealed that indeed it could, in addition to being able to develop concepts for human body parts and -- of course -- cats. Overall, there was 15.8 percent accuracy in recognizing 20,000 object categories, which the researchers claim is a 70 percent jump over previous studies. Full details of the hows and whys will be presented at a forthcoming conference in Edinburgh.

Marriage & My Common Good | Mere Orthodoxy | Christianity ...

I?ve recently returned from celebrating my wife?s grandparent?s 6oth wedding anniversary. Their marriage has been a picture of permanence, exclusivity, and joy.

My Granny and Gramps have three children, of which two are married. In total, they have four grandchildren. Three of the four grandchildren are now married. Two of the grandchildren have children of their own, thus making Granny and Gramps great-grandparents. From two persons, their marriage has resulted in no less than five marriages. The marriage of Granny and Gramps has, in the words of G.K. Chesterton, ?altered the destiny of nations? because theirs is a union that results in a good that will outlast the marital relationship itself. The ?good? I speak of, of course, is the stabilization and continuation of civil society. Society is little more than the mass aggregate that results from two persons duplicating conjugal relationships over and over.

As we sat around for a time of family worship on Sunday evening, it struck me that what I was witnessing was not merely a time of worship, but also a manifestation of the ?common good.? I looked around and saw not only a group of atomized individuals, but a large family, and smaller groupings of separate families. One initial family spawned more families. And so on. It?s in this ?common good? that I find the greatest apologetic for traditional marriage: The stability offered by the marriage of Granny and Gramps has resulted in marriages that, because of the model presented to their children during childhood, are themselves stable and mirroring the ideals of permanence, exclusivity, and the production of children. Stable marriages produce, typically, stable children. Stable children grow to participate in more stable societies. This is what we call human flourishing. It?s neither abstract nor difficult to embody. It?s as simple as what results from two individuals who loved each other and understood that the well being of their children mattered more than self-fulfillment.

So what?s the point?

Government has a compelling interest in promoting, protecting, incentivizing and validating relationships like that of Granny and Gramp?s because relationships like theirs secures the continuation and well being of the body politic. The United States government is dependent upon Granny and Gramps, not the other way around.?

If individuals reproduced asexually, marriage as an institution would not exist. People could collaborate, but there would be no innate incentive to restrict relationships around the concepts of permanence and exclusivity since the relationship lacks any form of permanent attachment or produced ?good? between the two peoples. Marriage exists because the sexual union between two persons can (and often does) produce children. Children need protected and the resultant child forms a necessary attachment between the individuals involved in a conjugal act.

Up to this point, I haven?t mentioned same-sex marriage, and I regret that I have to. I tire of the debate. You could have read this without same-sex marriage ever crossing through your frontal lobes. But I must mention it because the attempt to redefine the essence of marriage is a subtle, yet pernicious distortion about reality.?My goal has been, instead, to highlight the essence of marriage that cannot be mimicked apart from sexual complementarity: The production and rearing of children.

The marriage debates often take on highly philosophical arguments. It doesn?t have to be so. On ?a Sunday evening in mid-June, an act of marriage demonstrated a societal good. Thirteen people, I realized, owed either their lives or their marital bond to an act that occurred sixty years ago and which continues to occur up to present day. Same-sex couples cannot?by their very essence?recreate what I encountered on Sunday night. Same-sex couples can practice permanence and exclusivity, but they cannot by any biological act produce children. Same-sex couples are by definition childless. If marriage is a societal act that promises the continuation of society, any arrangement or sexual relationship that is childless cannot be societal in a foundational way.

I don?t want to devolve this essay into why same-sex unions are inferior to traditional marriage. It?s not, I?d argue, that same-sex couples are inferior to opposite-sex couples in feelings or devotion. It?s that government has no compelling interest to oversee or protect relationships that do not outlast the relationship itself. Same-sex couples exist in a relational vacuum that cannot, by definition, perpetuate its existence. None of this is written to suggest that same-sex couples ought not to have the right to be together nor live together, nor be happy. It?s to suggest that the same-sex relationship cannot ever be an actual marriage. For if Chesterton is right, same-sex relationships produce no nations to alter.

If you live on planet Earth and have access to a TV or radio, you are well aware of the story behind Adele's record breaking album 21. Just to make sure, Adele was so devastated after a breakup that, unlike most people who get dumped and turn into psycho stalkers, she decided to write an album with songs that could make the coldest hearted person cry like a teenage girl after getting dumped. This, however, is not the particular instance to which I am referring.

Adele is not the first person (and considering most songs are about being in love and heartbreak, and the remaining songs are about money or "swag," she won't be the last) to pour her heart out and express her emotions through her work, nor will she be the last person to turn personal heartbreak into fame and success. However, the person who has used his personal experiences as inspiration and not self-destruction, and who has had more of an impact on the music world than almost any other musician or artist is actually not a musician at all, but a philosopher. That's right; Friedrich Nietzsche, a nineteenth century German philosopher, probably deserves to win not only a "Lifetime Achievement Award" in music, but he is also deserving of dozens of Grammy's and an induction into every music genre's Hall of Fame.

What most people, and even most musicians, don't realize is that Nietzsche coined what can be considered one of the most used phrases in music and even society. In his book Twilight of Idols (1888), Nietzsche wrote "Was ihn nicht umbringt, macht ihn starker," which translates to English as "what does not kill me only makes me stronger." Not only is this phrase uttered and known by nearly every person in the civilized world, it has been the basis for more songs in more genres than any other phrase or quote (even more so than The Beatles' "All we need is love"). And, like Adele, Nietzsche's failures with love led him to adopt many of his philosophical views on love, life and even God.

Little did this "existentialist" know at the time, Nietzsche would be one of the most quoted people in history alongside Jesus and Confucius. He is not limited to being a "one hit wonder" of the quote world either; he has hundreds of gems and, for those of us who follow him on Twitter (@NietzscheQuotes), we enjoy his profound, pathetic and cynical words on a daily basis, and they are still extremely relevant. As a man who has had his share of rejection, I truly feel for him and his tortured soul. However, if his soul wasn't tortured, we wouldn't have the hundreds of songs that his quote has come to inspire.

So, thank you Friedrich Nietzsche, your words have helped and continue to help make artists such as Kelly Clarkson, Kanye West, Asia, Tupac and many more into award winners, and most of them I doubt would even know who you are. If only his family could find a way to cash in on the residuals and revenue acquired by using his iconic phrase? Whether it's Adele's song "Someone Like You," or Nietzsche's quote "Ah, women. They make the highs higher and the lows more frequent," it's safe to assume that the best revenge for being heartbroken is to become successful and famous, and the best way to do that is to write a song or something so profound someone will use it in a song. Though, I have also found that dating someone more attractive and successful than the person who dumped you works well too.

On a different note, for my superhero fans, Nietzsche also coined the term and concept of a "Superman," but I will save that for another blog entirely.

Summer Travels and Healthy Sleep for Your Baby | Baby Care Tips ...

Summer is here and warm weather means cottage, camping, and road trip time. A common question I get asked this time of year is ?We?re going away soon. How do I protect my baby?s sleep?? Here are tips on how to have a great summer vacation with a well-rested baby!

Consistent Bedtime Routine

Practicing a consistent bedtime routine at home is something you can take with you wherever you go. A similar sequence of events can better prepare your child for sleep. Practicing this routine away from home will help give the feeling of security and familiarity to your baby, even if they are unsure of their environment.

A Few of My Favorite Things

If your child has a lovey or blanket that is a comfort item, don?t forget to pack it! Giving them these items at bedtime, reading them familiar books, using the same white noise machine, can help ease the transition of a new sleep environment. If you are using a crib where you are going, use the same crib sheet from home so your baby will have that familiar smell. For your toddler, pack up their pillow case and use it on their new bed.

Take Your Time

If you are planning a long road trip try and leave around nap time so your little one can fall asleep in the car or plan your trip at night. If your baby is a car sleeper try and leave right at nap time. If your child has a hard time falling asleep in the car leave an hour before naptime so if he gets excited from the car he has an hour to relax and fall asleep and it won?t interfere with naptime. Also, give yourself some time to have a relaxed road trip for extra stops for feedings, potty breaks, and leg stretches.

Sleep Training

If you make the decision to start sleep training but you have an upcoming trip happening soon, I recommend holding off on sleep training. It?s so hard to be consistent when you are travelling and have jammed packed agendas. Consistency is key in any sleep training plan. So enjoy your trip and when you get back home you can get started right away.

If you have already sleep trained and are worried about keeping your little one on schedule try not to worry too much about it. You do want to protect their sleep as much as possible but you don?t want to be trapped inside during every nap. Try and encourage some quiet time and stroller sleep throughout the day if you can?t make it back to the hotel room or cottage for nap time. Try not to have a late bedtime every night. And most importantly when you get back home, get right back on track.

Suite Sleep is a Great Sleep

When booking your accommodations I?m going to recommend that you look at booking an economical suite with at least two separate rooms. Most suites have enough space in the main room for a pack and play and come with a sofa bed for toddlers. Some will even have another twin or double bed. Make sure you ask. This way mom and dad you can sneak off into the bedroom while your little one is falling asleep and you don?t have to hide out in the bathroom.

This summer enjoy your vacation time with your family. Be aware of how much sleep you child needs and try to protect it as much as possible but also have a great time doing family fun things and know that when you get back home, everything can go back to normal.
